Spinal Flow Wellbeing Clinic is trading name of Yogalates Therapy Ltd, company registration no. 15691389, whose registered office is 50E Roxburgh Terrace, Whitley Bay , NE26 1DS.

 

By engaging on this website and its 3rd party extensions and/or utilising any service provided wholly, or in part by Spinal Flow Wellbeing Clinic you agree to the Terms and Conditions set out below in terms of Use, Waiver and Privacy Policies, in addition to applicable laws and regulations such as GDPR and accept responsibility for compliance.

Intake Forms:

Intake forms are used to collect client information, insurance information, and consent before an appointment.

Waiver:

The intake form may also include a Waiver, depending on which service you have selected i.e., initial consultation, group healing sessions, or individual sessions. Consent forms are also attached to our intake form which relate to Privacy and Sharing of Information, Consent to Healing, and Cancellation Policy, prior to booking. Please see our Privacy Policy for further information.

Bookings: 

We use Jane App, for booking, data management, and collecting payments. Please refer to our Privacy Policy for further information about how your personal data is collected and managed. 

Payments:

We use Stripe, and Square to process and collect credit card payments. We use the Jane App for recording payment details which are encrypted. Receipts will be issued either via the Jane App, or Square.  We may on occasions accept cash.

Cancellation Policy:

There is a 24 hour Cancellation Policy, in which appointments can be cancelled. You will be charged 50% of the cost of the service appointment for not showing up or  cancelling outside of the appointment window. 

GDPR:

To request information on the details we have stored, please email spinalflowwellbeing@gmail.com with the subject heading "Subject Access Request." Please include the date you are making the request, your name, email address, customer account number, what information you want, the reason for wanting this information, and how you would like to receive this information. Please refer to our Privacy Policy for more information. 

Complaints & Queries:

Please email spinalflowwellbeing@gmail.com for any complaints or general queries. If you are complaining about an individual please give detailed information so that we can respond accordingly.
